- Obtain a COPY of the Veteran’s DD214* form (Please DO NOT send original and Mark out the Social Security Number on the DD214 before submitting).
- If paying by credit card, we request that the name on the credit card match the name of the “Requester” of the Paver Stone to keep our records straight.
