Example of 8” x 8” Paver Information for ordering:
- Complete Order Form (one for each Veteran Paver stone)
- Obtain a COPY of the Veteran’s DD214* form (please do Not send original). Please mark out the Social Security Number on the copy of the DD214 before submitting.
- Mail the completed Order Form, the copy of the Veteran’s DD214* form and the $200 payment by check or money order to: Murphy Veterans Tribute 120 E. FM 544 Suite 72, PMB 332, Murphy, TX 75094 – Checks and Money Orders to be made out to Murphy Veterans Tribute.

We request that the name on the credit card match the name of the “Requester” of the Paver Stone to keep our records straight.
*DD214 – A government document showing your Veteran’s Honorable Discharge from the Military Service.
